Overview
In Spain's public system, you don't freely choose any doctor from a national list — you're assigned a primary-care doctor (médico de cabecera) at the health centre covering your registered address. Private care works differently, giving you more direct choice.
Requirements
- For public care, you must be registered with your regional health service and hold a health card
- For private care, you need an active private insurance policy or be willing to pay directly
Step-by-step process
- In the public system, your primary-care doctor is automatically assigned based on your registered address (padrón) when you register for healthcare
- Visit or contact your assigned health centre (centro de salud) to confirm your doctor and register for their patient list
- For private care, search your insurer's provider directory (cuadro médico) or a clinic directly, since private doctors can be chosen freely
- If you're not comfortable with your assigned public doctor, see 'Changing your family doctor' for how to request a change
Required documents
- Health card (tarjeta sanitaria) for public care
- Private insurance card or policy details for private care
Common mistakes
- Assuming you can pick any public health centre freely — assignment is based on your registered address, not personal preference
- Not checking whether a private specialist is actually in your insurer's network before booking, risking an unexpected bill
Practical tips
- If you move address, your assigned health centre and doctor change too — update this alongside your padrón change
- Many private insurers publish an online directory letting you filter by speciality, language spoken, and location
